In the season 11 finale, Lady Toni Bevell, a London branch member of the Men of Letters managed to capture Sam. At that point though, Sam thought Dean was already dead which made him lose his will to fight.

Then a gun went off without revealing who was shot. Will the "Supernatural" season 12 premiere start with a gravely injured Sam?

The idea of the brothers fighting mortal enemies would be a fresh approach for the show. Fans have been used to the idea of the Winchester brothers going up against all sorts of supernatural entities.

While mortal enemies might be considered weaker than their supernatural counterparts, it does not mean that they are less deadly.
